Friends star Matthew Perry's official death certificate has been released by authorities.

The Los Angeles County Department of Public Health has made the document public nearly a week after he was laid to rest on Nov. 3.

In a file obtained by E! News, the actor's date of passing was on Oct. 28 at 4:17 p.m. in his residence at the Pacific Palisades outside LA. He was buried at the Forest Lawn Memorial Park, which is situated near the Warner Bros. Studios lot where his iconic 90s show was filmed.

His death certificate also revealed that there were no other significant conditions that contributed to his death.

Citing law enforcement sources, US media reported on Oct. 29 that Perry was found dead in a jacuzzi in his home. Entertainment outlet TMZ, in its report, noted that there was no foul play involved and that no drugs were found at the scene.

"A bystander had brought the man's head above the water and gotten him to the edge, then firefighters removed him from the water upon their arrival," LAFD Captain Erik Scott told E! News on Oct. 30

"A rapid medical assessment, sadly, revealed the man was deceased prior to first responder arrival," he added.

The five-time Emmy nominee was laid to rest last week. His funeral was attended by his nearest and dearest, including his mom Suzanne, his dad John Bennett Perry, and his stepfather, Keith Morrison.

A source also told PEOPLE Magazine that his Friends co-stars Jennifer Aniston, Courteney Cox, Lisa Kudrow, Matt LeBlanc, and David Schwimmer were also seen that day.

They said in a joint statement on Oct. 31 that they have been left "utterly devastated" by the sudden death of their co-star.

"We are all so utterly devastated by the loss of Matthew. We were more than just cast mates. We are a family," their statement read. "There is so much to say, but right now we’re going to take a moment to grieve and process this unfathomable loss."

"In time we will say more, as and when we are able," it continued. "For now, our thoughts and our love are with Matty’s family, his friends, and everyone who loved him around the world."

Friends ran on NBC from 1994 to 2004.

Apart from Friends, Perry also appeared in films like Fools Rush In and The Whole Nine Yards.
